Computer Service Pickup Request Form
Important Information
After submitting this form, you will receive an email. Please print out a copy of the message and tape it to the piece of equipment you need to have picked up and repaired so it can easily be identified.
There is a $39 minimum bench fee for all out-of-warranty equipment.
If your college/department would like to create a designated pickup/delivery point for your entire college/department, please consult with a computer service staff member.
